2. How to Get a School Physical at HOPE

HOPE Family Care Center operates on a first-come, first-served basis during clinic hours, so there’s no need to call ahead or schedule an appointment. Simply visit during our clinic hours, and we’ll help you with your child’s school physical.

  • Clinic Hours: 1st and 3rd Saturdays 9:00am - 11:30am AND 2nd and 4th Tuesdays 6:00pm - 8:30pm

  • What to Bring: Be sure to bring any necessary school forms, your child’s immunization records, and any other relevant medical history.

Our team will complete the required physical and any forms your school needs for your child to participate in sports, activities, or classes.
